Phoenix Public Library: Chronic Disease Self-Management Workshop

This is a workshop for adults with at least one chronic health condition, which may include arthritis. It focuses on disease management skills including decision making, problem-solving, and action planning. Presented by Area Agency on Aging.
To successfully complete the program, participants will attend at least four of the six sessions.
Attendance and all program materials are free of cost.
Participants must be a Maricopa County resident, and 60 years of age or older.
Registration is required.
